scarless blepharoplasty
Scarless Lower Blepharoplasty
Removal of fat pockets through a transconjunctival approach, with no visible scar on the eyelid.
Duration
45-90 minutes
Anesthesia
Local anesthesia with sedation
Result
Lasting result 10-15 years; removed fat does not return
Overview
Scarless lower blepharoplasty uses the transconjunctival approach β an incision made on the inner surface of the lid, in the conjunctiva β providing direct access to the fat pockets with no external scar. This technique is ideal for younger patients with fat pockets and no significant skin excess.
The major advantage of the transconjunctival approach is the complete absence of any visible scar and full preservation of the lower lid musculature, virtually eliminating the risk of ectropion. Dr. Mubariz Mammadli combines this technique with fat repositioning into the tear-trough to simultaneously correct structural dark circles.

Frequently asked questions
How can there be no visible scar?
Incisions are made on the conjunctiva β the inner lid mucosa. This area leaves no scar and heals spontaneously in a few days. No trace remains on the external side.
Who is the ideal candidate for this technique?
Younger patients with fat pockets and elastic eyelid skin. Patients with significant skin excess need a standard blepharoplasty with subciliary incision.
How does recovery compare to classic blepharoplasty?
Recovery is much faster β return to social activity in 5-7 days versus 10-14 days for classic blepharoplasty. Swelling and bruising are far more discreet.
Is the risk of ectropion lower?
Yes, significantly. The transconjunctival technique does not disrupt the lower lid musculature or tone, virtually eliminating the risk of postoperative ectropion or scleral show.
Do the bags come back over time?
Removed fat does not regenerate. With age, minor new accumulations may appear, but the treated appearance remains significantly improved long-term.
